Program Guidance
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with any dental insurance plans
- For treatment which, in the sole opinion of our doctors, lies outside of the realm of their capability.
- For hospital charges of any kind.
- For costs of dental care that are covered under automobile, medical, or workers' comp.
- For referrals to specialists.
*The plan is a yearly commitment. If the participant decides not to utilize this membership plan, it is non-refundable. If the plan lapses, a reactivation fee will be required. Monthly or annual payments will be automatically deducted from your credit card.
