This program is a discount plan, not a dental insurance plan. It cannot be used:

In conjunction with any dental insurance plans
For treatment which, in the sole opinion of our doctors, lies outside of the realm of their capability.
For hospital charges of any kind.
For costs of dental care which are covered under automobile, medical, or workers' comp.
For referrals to specialists.

*The plan is a yearly commitment. If the participant decides not to utilize this membership plan, it is non-refundable. If the plan lapses, a reactivation fee will be required. Monthly payments will be automatically deducted from your credit card.
